Metabolic Rehabilitation
Obesity, Diabetes, and other related cardiometabolic diseases are preventable diseases that are caused by multiple factors and require a multi-factorial treatment plan. Medication alone doesn’t address these factors enough to help persons living with these diseases or at risk of developing these diseases manage or prevent these cardiometabolic conditions. H.E.A.L. Mississippi’s Metabolic Rehabilitation Program is a multi-factorial treatment plan that addresses all factors that contribute to developing these conditions. Our program consists of exercise therapy, obesity counseling, nutritional counseling, behavioral counseling, chronic disease management education, diabetes education, physical and occupational therapy, basic pharmaceutical education, dietary assessments and related medical services.
MEND
Mind Exercise Nutrition Do It! (MEND) is an international public health program for children with or at risk of obesity and their families aimed at disease prevention and overall healthier living. MEND provides the necessary support to achieve optimal health and maintain healthy lifestyles for children and their families. With effective behavior strategies employed to empower families to change and make better choices, families learn to enjoy a healthier way of living, motivating them to be more physically active while helping them discover that healthier foods and drinks can be delicious.
National Diabetes Prevention Program (NDPP)
The National Diabetes Prevention Program is a program aimed to prevent type 2 diabetes. The NDPP was created in 2010 to address the increasing burden of prediabetes and type 2 diabetes in the United States. This national effort created partnerships between public and private organizations to offer evidence-based, cost-effective interventions that help prevent type 2 diabetes in communities across the United States. The program is a yearlong group intensive that discusses various topics that will help prevent type 2 diabetes. Outcomes expected throughout the duration of the program are weight reduction, increased physical activity levels, and applicable knowledge to make healthier choices.
Diabetes Self-Management (DSMES)
Diabetes Self-Management Education and Support program (DSMES) provides an evidence based foundation to empower persons with diabetes to navigate self-management decisions and activities DSMES is a cost-effective tool proven to help improve health behaviors and health outcomes for persons with diabetes. The program is a 6 week intensive with yearly follow-up and also follow-up as needed but patients have access to their diabetes care specialist as needed to help aid in self-management decisions.
Band C.H.A.M.P.
Community Health Ambassadors for Minorities Program (Band C.H.A.M.P.) is a multi-level intervention that combines peer-to-peer health education among students designed to increase health literacy, disease risk awareness, healthcare system basics, reverse projected preventable chronic cardiometabolic disease trend, and decrease barriers to achieving optimal health. According to previous reports from the CDC, 1 out of 2 minorities born year 2000 and after, are projected to develop type 2 diabetes sometime during their lifespan. Band C.H.A.M.P was developed to intervene in these projections. Ambassadors are selected and educated on preventable chronic diseases such as obesity and diabetes, and they creatively and innovatively deliver this pertinent evidence-based disease prevention information to their peers. Health ambassadors are trained by licensed health practitioners on administering certain health screens like checking weight and height to calculate BMI, measuring body circumference to perform body fat analysis, and conducting blood pressure checks where the perform health screening clinics at various times during school year.
Preventive and Primary Care Services
H.E.A.L. Mississippi Preventive and Primary Healthcare services provides treatment for common illnesses via health promotion, disease prevention, health maintenance, patient counseling, patient education, diagnosis, and treatment of acute and chronic illness. The COVID-19 pandemic allowed us to expand our practice to provide relief for communities throughout Mississippi disproportionately effected by COVID-19. In early 2021, we became a COVID-19 vaccination provider and partnered with the Mississippi State Department of Health in delivering COVID-19 vaccinations to long-term care facility residents and homebound residents throughout Central Mississippi. Home visits have allowed us the opportunity to expand our telehealth services and now we are able to meet persons where they are most comfortable and provide them the high quality care they expect.
Our slogan “ You Can’t Spell Health with out HEAL” is meant to be taken in the literal sense as we aim to lift any barrier necessary to provide easily accessible and high quality care, the type of care the people expect from their healthcare team.
